PalliativIntens is an interdisciplinary research group led by Dr. med. Hanne Lademann, M.D., consisting of specialised physicians and medical students from Rostock University Medical Centre. This includes the head of the department of  pediatrics, Prof. Dr. med. Jan Däbritz (M.D.), Prof. Dr. med. Carl Friedrich Classen (M.D., head of the section paediatric oncology, haematology and palliative care), Prof. Dr. Peter Kropp (director of the Institute for Medical Psychology and Medical Sociology) as well as PD Dr. med. Johannes Ehler (M.D., senior physician in the paediatric intensive care unit) and PD Dr. med. Olbertz (head of the department of neonatology at the Südstadt Hospital Rostock). External specialists are consulted for specific questions.

After medical school, Dr. Hanne Lademann worked for three years in the neonatal and paediatric intensive care unit. She is funded in the Clinician Scientist Program of the University Medical Center Rostock. 

The aim of the research group is to evaluate and improve the care of seriously ill children in the neonatal and paediatric intensive care unit (initially) in Rostock. Here, the focus is particularly on patients for whom a (partially) palliative therapy approach should be considered due to their diagnosis and/or the course of their disease.

"PalliativIntens Team"

The project "PalliativIntens Team" is currently being carried out. It is funded by the FORUN-Program and the Clinician Scientist Program of the University Medical Center Rostock.

Background

Approximately 40% of all deaths in childhood occur in the neonatal period. Neonates make up about 80% of this group and die predominantly in the neonatal intensive care unit. Due to the constant development of medical possibilities, not only the probability of survival in neonatal (NICU) and paediatric intensive care units (PICU) is increasing, but also the morbidity of the affected patients. The integration of a palliative attitude and practice into paediatric and neonatal intensive care is becoming increasingly important (1-3). Scientific publications on this topic have been written for about 20 years, with a focus on reviews, expert opinions and experience reports (4-6). To date, few intervention studies have shown significant improvements in palliatve care by implementing aprotocols, standard operation procedure or palliative care counsils (7-10).

Aim

To assess the effect of a standard operating procedure (SOP) on barriers, moral distress, attitudes towards and practice of care for palliative care patients in the local NICU, PICU and adult intensive care unit (AICU). The focus of the project is therefore on the team.

Methods

First of all, the scope of palliative care is determined within the framework of a retrospective analysis of the courses of critically ill patients in the NICU and PICU in Rostock on the basis of defined endpoints. Quantitative staff surveys (physicians, nurses, psychosocial service of the NICU, PICU and AICU) are planned to record the moral disstress caused by, attitudes towards and barriers to palliative care of seriously ill patients. In addition, tailor-made SOPs are developed on the basis of a systematic literature research and according to a qualitative staff survey (semi-structured individual interviews and content analysis (physicians, nursing staff, psychosocial service of the NICU, PICU and AICU)). One year after implementation of the SOP practice, attitude and stress will be conducted again.

Staff of the NICU and PICU of Greifswald University Medical Centre and the Children's Hospital Wilhelmstift Hamburg, Germany will be involved as control groups for the items moral disstress, attitude and barriers.

"PalliativIntens Kids&Parents"

The follow-up project after PalliativIntens Team is currently being conceptualised. In the preliminary work, we were then able to show whether a SOP for palliative care in a neonatal, paediatric and adult intensive care unit could reduce the moral distress of staff, influence their attitudes and improve the hollistic quality of care for critically ill patients.

Aim

To assess the effect of a national guideline on the symptom burden of children and the psychological distress, anxiety and quality of life of mothers/parents in the NICU. The focus of the follow-up project is therefore on the children and parents.
